Ketentuan Kelengkapan Dokumen Lapor Diri\ud83d\udccc<\/p>\n

Ketentuan Kelengkapan Dokumen Lapor Diri Peserta PPG Dalam Jabatan Tahun 2023 Batch 1 LPTK IAIN Syekh Nurjati Cirebon, sebagai berikut :<\/p>\n

1. Format A1 dan Pakta Integritas (PI) (format diunduh dari SIMPKB);<\/p>\n

2. Biodata (sesuai format PD DIKTI);<\/p>\n

3. Surat Pernyataan Izin Kepala Sekolah untuk mengikuti PPG (format bisa diunduh dari SIMPKB\/Siaga\/Simpatika);<\/p>\n

4. Foto Copy Ijazah S1 (terakhir);<\/p>\n

5. Foto Copy Transkrip Nilai S1 Legalisir;<\/p>\n

6. Foto Copy Kartu Identitas KTP\/ SIM;<\/p>\n

7. Pas Foto Berwarna 4×6 (background merah);<\/p>\n

8. SKCK (dari Kepolisian setempat);<\/p>\n

9. Surat Keterangan Sehat (SKS) (dari Puskesmas\/RSUD setempat);<\/p>\n

10. SK Kepegawaian terakhir.<\/p>\n

Dokumen dikirim Ke Fakultas Ilmu Tarbiyah dan Keguruan IAIN Syekh Nurjati Cirebon dengan di tujukan kepada<\/p>\n

*Panitia PPG Dalam Jabatan Tahun 2023 atas nama Bapak Dr. Muslihudin, M.Ag*<\/p>\n

diserahkan Paling lambat pada Hari\/Tanggal : *Jum’at, 12 Mei 2023*<\/p>\n
